A handful of supplements have real evidence for lowering biological age, not just slowing it. The list is shorter than the marketing suggests, and it looks nothing like the long influencer stacks. Most products sold for anti-aging have never been measured against an aging biomarker in people. A few have, and a couple show measurable reversal. The rankings and review numbers below come from Reverse Age Lab's research engine, which scores each supplement by pairing its human-study evidence with verified user reviews. These are the reverse aging supplements with human trials behind them.
The short answer: only a few supplements have been measured against an aging biomarker in people. Calcium alpha-ketoglutarate has the most direct signal, GlyNAC, nicotinamide riboside with pterostilbene and urolithin A have randomized human trials on aging markers or function, and NMN alone, resveratrol and spermidine are still lab interest without human proof. Match the studied brand, formula and dose to your own profile.

Key takeaways
Only a few supplements have been measured against an aging biomarker in people.
Calcium alpha-ketoglutarate has the most direct signal on biological age.
GlyNAC, nicotinamide riboside with pterostilbene and urolithin A have randomised human trials on aging markers or function.
NMN alone, resveratrol and spermidine remain laboratory interest without human proof.
Match the studied brand, formula and dose, because the trial was run on a specific product.
None of them replaces exercise, sleep and diet.
Can Supplements Reverse Biological Age?
For a small group, yes, and the proof is human, not theoretical. Biological age is measured with DNA-methylation epigenetic clocks and related biomarkers, and a few supplements have moved those markers in people. The clearest case is calcium alpha-ketoglutarate. In a retrospective human study using DNA-methylation clocks, adults who took Rejuvant for about seven months showed an average biological age reduction of roughly eight years. That is reversal, measured on an accepted biomarker, in humans. Most supplements never get tested this way, which is why so much marketing leans on lab dishes and mice. The seven below are the exceptions worth your money. The clocks themselves rest on the multi tissue DNA methylation work of Horvath (2013), and the Rejuvant result is a retrospective study of 42 people rather than a randomized trial (Demidenko et al., 2021), which is why it is a strong signal and not a settled fact.
Supplement | What was measured in people | Strength |
|---|---|---|
Calcium alpha-ketoglutarate | Average drop in biological age on DNA methylation clocks | Most direct signal, retrospective |
GlyNAC (glycine plus NAC) | Strength, cognition and metabolic markers in older adults | Randomised, small |
Nicotinamide riboside with pterostilbene | Raised NAD significantly | Randomised, marker only |
Urolithin A | Muscle endurance in older adults | Randomised, functional outcome |
NMN alone | Raised NAD, modest walking distance change | Early |
Resveratrol | No link to lower mortality or disease | Not supported |
Spermidine | Intake associated with survival in a cohort | Observational only |
The Reverse Aging Supplements With Real Human Evidence
Rejuvant LifeTabs, built around calcium alpha-ketoglutarate, is Reverse Age Lab's top reverse-aging pick at Tier 1. The retrospective human study above found an average biological age reduction of about eight years on DNA-methylation clocks. Across 75 reviews it holds 99% positive sentiment, with users reporting steady energy and a sense of slowed aging.
GlyNAC, the combination of glycine and N-acetyl-cysteine, is Tier 1. Human studies show it corrects the age-related glutathione deficiency behind oxidative stress and inflammation, with measured gains in muscle strength, gait speed, cognition, and metabolic markers in older adults. It is the most reviewed pick here at 96% positive across 19,702 reviews, where people cite more energy and faster recovery. (Kumar et al., 2023.)
Elysium Basis pairs nicotinamide riboside with pterostilbene, also Tier 1. A human clinical trial showed it safely and significantly raises NAD, the cellular coenzyme that falls with age, in older adults. Reviews run 88% positive across 1,372, with users describing better daily energy and clearer thinking. (Dellinger et al., 2017.)
Mitopure, a purified form of urolithin A, is Tier 1. Human evidence shows it improves muscle endurance and strength in middle-aged and older adults by renewing mitochondria, the cell's energy producers. Reviews are 83% positive across 1,589, with a common theme of gradually improved stamina. (Liu et al., 2022; Singh et al., 2022.)
SRW Cel1 is Tier 1. Its formula targets the oxidative stress and inflammation behind age-related cellular decline, supporting cellular stability and repair. Reviews are 96% positive across 72, with users noting a real difference in wellbeing after a few weeks.
Niance Epigen12 is Tier 1. The maker reports biological age and cellular gains within twelve weeks, targeting epigenetic aging and mitochondrial function. The human data here is brand-supplied rather than independently published, so weigh that claim more cautiously than the others. Reviews are 100% positive across 29, centered on visible skin rejuvenation.
Earth Harmony Ultimate Methylene Blue is Tier 1. Pharmaceutical-grade methylene blue supports mitochondrial energy and carries human clinical evidence in cognitive and cardiovascular health. Reviews are 80% positive across 416, the most mixed of the group, so source and dose matter. The human brain imaging evidence comes from a randomized functional MRI study (Rodriguez et al., 2016).
What the Evidence Does Not Yet Support
NMN on its own gets bold claims, but outside the nicotinamide-riboside-plus-pterostilbene trial above, most reversal data is still animal work. Resveratrol and spermidine sit in the same place: real laboratory interest, thin human proof. They are worth watching, not worth building a protocol around yet, and the same goes for the long influencer stacks, where most ingredients have never been measured against an aging biomarker in a single person. Sources: the NMN trial in prediabetic women (Yoshino et al., 2021), the resveratrol cohort with no mortality link (Semba et al., 2014) and the spermidine intake study (Kiechl et al., 2018). How to Choose
Two supplements with the same name are not the same product. The evidence is tied to a specific brand, formula, and dose, the version that was actually studied. Match the supplement to your own biology and goals rather than copying someone else's routine, and prioritize the picks with human biomarker data over those with only lab or animal research.
The best results come from matching the right evidence-backed supplements to your own profile. Reverse Age Lab does not sell supplements. It reads the human studies and the real reviews for your specific goals and builds the protocol for you, so you can stop guessing from influencer stacks. How each product earns its tier is on the methodology page.
Ready to see which fit you? Rala grades every product on the human studies and real reviews and builds one daily protocol around your goals and labs. Reverse Age Lab sells no supplements. Start your 7 day free trial, or check any product with the free research tool.
